Click on the links below to download registration forms for the 2020-2021 school year. If you are unable to download and print these forms, they will be available for you at registration.
If your children are currently enrolled at Springville, you must register them online. Online registration will be open and available Tuesday, August 4th. Please DO NOT fill out the K-12 registration form.
Please download and fill out the K-12 registration form if your child is new to the district, or is an incoming Kindergarten student Please download the optional forms that pertain to your children, and return them to the main office at the secondary.

**Forms with a * next to them are required permissions. If you are new to our district, please print these forms and bring them to registration along with the Registration form above.

Athletic physicals are good for one calendar year + 30 days. Athletes are unable to participate without a current physical. If you have questions about an athlete's physical please contact Joe Martin @ jmartin@springville.k12.ia.us.
This is a reminder that Iowa Code Section 280.13C requires that annually, every student (grades 7-12) and their parent/guardian must receive and sign the concussion and brain injury information sheet titled, "Heads Up: Concussion in High School Sports" before the student is able to participate in interscholastic sports, cheerleading, and dance in any way (practice or competition). Any forms signs previously are valid for 12 calendar months from the date of signature, then, signatures must be obtained again for the student to continue participating. There is NO GRACE period for the "Heads Up: Concussion in High School Sports" form.
